Наоборот же.
"Подстановка" A вместо B (замена B на A).
AHE вот только говорит, что в спорте, когда игроков заменяют так говорят, когда имеется в виду замена A на B. В остальных случаях после for идёт то, что заменяется, а не то, чем заменяется.

I don't think so. If you try to translate it the other way round, the referencing/substitution (ha :) doesn't work.
Who's this then? The sentence only works if Palmer = he.

And in all of them (including yours), the meaning's crystal clear from the context.

Besides, this incorrect usage pattern in a dictionary is not something a learner should be exposed to.

Because "substitute X for Y" means the opposite of "substitute X with Y". Btw, many dictionaries object to the second pattern (and suggest using "replace" instead), but at least there's the preposition to differentiate between the two meanings. Yeah, that's confusing, but that's how language works -- it's a living organism, so sometimes it does get confusing. If it's too confusing, at some point one of the meanings will naturally disappear. As of now, I've come across both instances of usage.

That’s fine if you put it in two sentences like that, it becomes clarified by context. But too many people describing recipes will get it wrong where the context really does matter. For example:
I used my grandmother’s recipe, but I substituted apples for pears.
That sentence above is totally unclear, particularly if you don’t have access to Grandmother’s recipe. This should correctly mean that Grandma specified pears, but I used apples instead. But 80% of people would interpret this the opposite way. The result is confusion.

В обоих случаях ставим А вместо В. Просто в первом случае глагол, поэтому там предлог of не нужен.
