Интересно, что во
втором издании (unit 14) ниже этого процитированного места из
третьего издания есть дополнение, что в менее формальных контекстах
неперфектный инфинитив тоже может употребляться для выражения того, что не произошло -
- was/were to + infinitive can be used informally to talk about things that didn't happen:
We were to see each other that day, but I had to phone and cancel/ (or We were to have seen/We were supposed to see...)
Этого дополнения

• I was to have helped with the performance, but I got flu the day before.
• There was to have been a ban on smoking in restaurants, but restaurant owners have forced the council to reconsider.
When we use was/were to + infinitive we are talking about something that did actually happen.
When we use was/were to have + past participle we are talking about something that had been arranged, but did not happen.
Но самое забавное, что в
третьем издании (unit 14) это добавление было снова удалено)) - осталось только упоминание про
be supposed to:
...in less formal contexts we would often more naturally use be supposed to:
I was supposed to help, but I was ill. (more natural than I was to have helped...)
